Your Rights
When receiving DDRB case management services you are entitled to the following rights without limitation:
- To be treated with respect and dignity as a human being;
- To have the same legal rights and responsibilities as any other person unless otherwise limited by law;
- To have the right to due process review when any limitation to your rights is proposed or is alleged to have taken place;
- To receive services regardless of your gender, race, creed, marital status, national origin, disability or age;
- To be free from physical, verbal, mental and sexual abuse and neglect;
- To be free from chemical and physical restraint;
- To participate in or refuse to participate in research projects;
- To receive appropriate humane and high quality services and supports as determined by your support team, which may include, but not be limited to, you, your parents, your guardian or other authorized representative;
- To receive these services and supports in the most integrated setting appropriate for your particular needs;
- To have access to DDRB rules, policies and procedures pertaining to services and supports;
- To have access to your personal records;
- To have your personal records maintained confidentially; and
- To have services, supports and personal records explained to you so that they are easily understood.
Due Process
- You have the right to be heard or have someone you choose speak for you before your rights or services can be limited or taken away.
HIPAA Privacy Rights
You are entitled to the following rights regarding your personal medical/health information:
- To see and get a copy of your record;
- To appeal if the DDRB decides not to let you see all or some parts of your record;
- To request your record be changed due to an error or incomplete information;
- To know whom your information has been sent to;
- To limit how the DDRB uses or discloses information about you;
- To ask that we communicate with you about medical matters in a certain way or at a certain location.
- To authorize other releases of your personal information;
- To remove the authorization at any time;
- To have a paper copy of the Notice of Privacy Practices;
- To file a complaint if you believe any of your rights have been violated.
All requests must be in writing.
